Patient Forms, Map and Directions to our Two Plymouth Offices

Your first visit to Kerr Dental Associates establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Mission Statement
Our practice is working together to realize a shared vision of uncompromising excellence in dentistry.

To fulfill this mission, we are committed to:

  • Listening to those we are privileged to serve.
  • Earn the trust and respect of patients, profession and community.
  • Exceed your expectations.
  • Ensure a creative, challenging and compassionate professional environment.
  • Strive for continuous improvement at all levels.

What To Expect
We think we have created a very welcoming, caring and comprehensive practice and we think you will too.

If you have x-rays at a previous dentist, please have them forwarded to our office.  Call us and we can help you with this.  Full mouth x-rays are required for all patients.  We take a Panoramic only once every 5 years and bite wing x-rays generally once a year.

The first visit for an adult new patient is with the doctor for approximately 90 minutes.   At this first visit the doctor will first talk with you to understand what your priorities are for your oral health and your smile.  The doctor will then chart all your existing teeth and fillings for our records.  A comprehensive periodontal (gum) exam is also completed and an oral cancer screening is provided.   At this point the doctor can then determine any restorative work that may be needed.  At this time a cleaning is then scheduled.  Some patients need an hour for a cleaning and other patients may need 90 minutes.

We all look forward to meeting you.

Drs. Stephen J. Kerr,  J. Devon Lutz, D. Patricia DeVore and Renee L. DiDonato
